ABSTRACT

OBJECTIVE To extract the effective components of Psoralea corylifolia and evaluate its efficacy in the treatment of vitiligo. METHODS The concentrations of psoralen, isopsoralen, neobavaisoflavone, corylin, psoralidin, corylifolinin, and bakuchiol in P. corylifolia extract were determined by ultra-performance liquid chromatography. Based on the analytic hierarchy process (AHP) and Plackett-Burman design, with the concentrations of the 7 components as evaluation indexes and the crushing degree, ethanol concentration, and soaking time as factors, the extraction process of P. corylifolia was optimized by Box-Behnken response surface methodology and the validation test was conducted. Zebrafish were divided into blank control group, positive control group (8-methoxypsoralen, 10.8 μg/mL), and low-, medium-, and high-concentration groups of P. corylifolia extract (500, 1 000, 2 000 μg/mL), with 6 fish in each group. The effects of P. corylifolia extract on the melanin production of zebrafish were studied by density analysis. RESULTS The best extraction process was P. corylifolia powder over 60 meshes and soaked in 80% ethanol for 72 hours. The average comprehensive score of three validation experiments was 98.27, with an RSD of 1.36%, and the relative error was 1.02% compared with the predicted value of the fitting equation (97.28). Compared with the blank control group, the melanin pigmentation of zebrafish in the low-, medium-, and high-concentration groups of P. corylifolia extract was significantly increased (P<0.01). CONCLUSIONS The optimized extraction process of P. corylifolia is reasonable and feasible, and the obtained P. corylifolia extract can significantly promote the production of melanin in zebrafish.

ABSTRACT

Objective:To introduce a newly modified surgical approach and explore the clinical efficacy of en bloc kidney transplantation from deceased infant donors to adults.Methods:Four cases of en bloc kidney transplantation from deceased infant donor to adult were performed with a new modified surgical approach in renal transplantation and dialysis center of the Second People's Hospital of Shanxi Province from January 2017 to September 2019. All 4 cases were cardiac death donors. There were 3 males and 1 female donors, aged (54 ± 22.69) d and weighing (5.6 ± 0.79) kg. There were 1 male and 3 female recipients, aged (41.5 ± 5.97) years and weighing (45 ± 3.56) kg. For lowering operative difficulties and preventing hemodynamic disturbances, bilateral kidneys were sutured and fixed during trimming. Then end-to-side anastomosis was performed between donor kidney abdominal aortic valve and recipient external iliac artery and between donor renal inferior vena cava valve and recipient external iliac vein.Results:All operations were successful without vascular or urinary complication. The average duration of donor kidney repair was 20 min and the average duration of transplantation 68.75 min. The functions of transplanted kidney recovered well during a follow-up period of 12 months. The long-term survivals of recipient and transplanted kidney were satisfactory.Conclusions:The newly modified surgical approach of en bloc kidney transplantation from deceased infant donor to adult has optimized operative handling. With a higher success rate, vascular complications are lowered. A wider popularization is recommended.

ABSTRACT

Objective:To study the changes of food characters in early swallowing recovery in patients with different laryngeal function preservation surgery.Methods:Collected patients with laryngeal cancer hypopharyngeal cancer who underwent laryngeal function preservation surgery in Lihuili Hospital of Ningbo Medical Center from January 2019 to March 2020. By fiberoptic endoscopic evaluation of swallowing (FEES) combined with Penetration and Aspiration Scale (PAS), prospectively observed the aspiration and invasion of solid, paste, fluid and other food in the early stage of trial feeding in patients with laryngeal hypopharyngeal cancer undergoing open laryngeal function preservation.Results:Among the 69 patients, 21 had vertical partial laryngectomy and 19 had partial pharyngeal partial laryngectomy. 15 days after the operation, 2 groups of patients after solid and paste food adaptability scored (1.14±0.36), (1.29±0.56) and (2.53±2.04), (2.84±2.31) points, Friedman Mtest for comparison difference had statistical significance ( Mvalues were 23.463 and 22.227, P<0.01); the liquid food for adaptability scored (2.10±1.09), (4.42±2.24) points, the pairwise comparison of liquid, solid and paste showed statistically significant differences ( tvalues were-0.976 to 1.105, P<0.05). The pairwise comparison of the adaptability of the two groups on 20 days after surgery showed no statistically significant difference ( P>0.05).Throat on the cricoid cartilage resection-ring hyoid epiglottis anastomosis (SLCP-CHEP) was 17 cases, laryngeal glottis level partial resection was 12 cases; postoperative 15 days the adaptability on the behavior of three kinds of food grade 2 groups of patients (4.65±1.90), (5.59±1.46), (6.53±1.13) points and (6.67±1.07), (4.50±2.07), (6.92±0.79) points, respectively; Minspection differences were statistically significant ( Mvalues were 29.525, 22.136, P<0.01).The pairwise comparison of solid and paste food in the two groups 20 days after the operation showed no statistically significant difference ( P>0.05), while the difference of liquid and paste was statistically significant ( tvalues were-1.375 to-0.853, P<0.05). Conclusion:In the early recovery of patients undergoing laryngeal function preservation surgery, the vertical group and the partial pharyngeal partial laryngectomy group has better adaptability to solid and paste food. The horizontal group has better adaptability to paste, the CHEP group has better adaptability to solid, and the four groups has the worst adaptability to convection, the recovery time of convective mass between the CHEP group and the horizontal group was longer. To understand the adaptability of patients with different surgical methods to food traits at the early stage of trial feeding can help to implement targeted rehabilitation programs, carry out progressive diet training, and reduce the complications of misinvasion, aspiration, aspiration pneumonia and other complications in the recovery cycle.

ABSTRACT

OBJECTIVE:To analyze the mass spectrometry fragmentation regularity of bicyclol ,bifendate and schisandrin C , and to identify the impurities of bicyclol raw material. METHODS :UHPLC-ESI-Q-TOF-MS method was adopted. Using electrospray ionization source ,in positive ion mode ,the excimer ion and characteristic fragments of bicyclol ,bifendate and schisandrin C were analyzed by means of TOF-MS. According to the mass change of fragments ,the possible fragmentation pathways were speculated ,and the fragmentation regularity were analyzed and summarized. Bicyclol raw material sample was first separated by liquid chromatography to find the impurity peaks in it ,and then the impurity peaks were analyzed by mass spectrometer;the impurity identification was conducted by combining with the fragmentation regularity. RESULTS :The 3 compounds all produced [M+H] + excimer ion in the positive ion mode. After collision-induced dissociation ,the C-O bond ,the simple rupture of the C-C bond and the ring-opening cleavage of the oxygen ring occurred ;with the loss of neutral fragments , mainly CH 2O,supplemented by CO 2,CO and CHO ,the dissociation was concentrated in the middle and high quality regions. C-C and C-O bonds of 3 compounds were simply broken only in the branched chain structure and/or oxygen ring structure ,but the structure of the biphenyl parent nucleus remained unchanged. Among them ,the bicyclol contained a benzyl alcohol structure ,so under acidic mobile phase conditions ,it would exist stably in the form of [M+H -H2O]+. Because schisandrin C contained 8-membered ring structure,ring opening first occurred under collision voltage ,and then neutral fragment loss occurred. The secondary mass spectra of impurity in bicyclol raw material were consistent with the mass spectra fragmentation of secondary fragments of bifendate. CONCLUSIONS:The study summarized mass spectrometry fragmentation regularity of 3 schisandrin derivatives. The impurity in bicyclol raw material may be bifendate.

ABSTRACT

OBJECTIVE:To establish a method for concentration determination of anlotinib in human plasma and apply it in the clinic. METHODS :The plasma samples were pretreated by salting-out assisted with liquid-liquid extraction with ammonium acetate as salting out assistant and acetonitrile as solvent. Using voriconazole as internal standard ,LC-MS/MS method was adopted. The separation was performed on Waters X Bridge C 18 column with mobile phase consisting of 0.2% formic acid solution- acetonitrile(gradient elution )at the flow rate of 1 mL/min. The column temperature was set at 40 ℃,and sample size was 10 μL. The split ratio was 3∶7. The electrospray ion source and multiple reaction monitoring mode were used for the analysis. The ion pair of anlotinib and internal standard under positive ion mode were m/z 408.3→339.3 and m/z 350.2→281.3,respectively. RESULTS : Anlotinib showed a good linear relationship in the concentration range of 0.2-200 ng/mL(R2>0.996 7). The lowest limit of quantitation was 0.2 ng/mL. Intra-day and inter-day RSDs were no more than 12% (n=6 or n=3). Accuracies were 90.92%-108.00%(n=6 or n=3). The average extraction recoveries were 87.51%-100.00%(RSD<8%,n=6). The average matrix effects were 96.66%-99.93%(RSD<5%,n=6). The plasma concentration of 3 patients with NSCLC treated with anlotinib was 8.74-65.60 ng/mL. CONCLUSIONS :The method is simple ,accurate and specific ,and is suitable for the plasma concentration monitoring of anlotinib in NSCLC patients.

ABSTRACT

OBJECTIVE:To study the effects of rat intestinal flora on the pharmacokinetic parameters of pyrazinamide and its active metabolite pyrazinoic acid. METHODS :Totally 16 SD rats were randomly divided into trial group and control group ,with 8 rats in each group. Trial group was given mixed antibiotics (streptomycin sulfate+neomycin sulfate )intragastrically to construct pseudoaseptic rat model. After modeling ,both groups were given pyrazinamide intragastrically (150 mg/kg). Before and 0.167, 0.333,0.667,1,1.5,2,3,4,6,9 h after administration ,0.1 mL blood sample was collected from orbital venous plexus ,and 0.3 mL blood sample was collected from orbital venous plexus 12,24 h after administration. Using phenacetin as internal standard , LC-MS/MS method was adopted to determine the plasma concentration of pyrazinamide and pyrazinoic acid. The determination was performed on Agilent ZORBAX SB-Aq column with mobile phase consisted of 0.2% formic acid (containing 8 mmol/L ammonium acetate)-methanol(gradient elution )at the flow rate of 1 mL/min. The column temperature was set at 30 ℃,and sample size was 10 μL. The ion source was ESI and the temperature of ion source was 500 ℃. The collision gas was nitrogen and the pressure was 10 psi. The temperature of mass transfer interface was 100 ℃. The mass spectrum monitoring mode was multi reaction monitoring , and the collection mode was positive ion mode. The monitoring transition ion-pairs were m/z 124.0→79.0(pyrazinamide),m/z 125.1→79.1(pyrazinic acid )and m/z 180.0→110.2(internal standard ). The de-clustering potential and collision voltage were 55, 26 and 85 V,24,23 and 28 V,respectively. The pharmacokinetic parameters were calculated and compared by using DAS 2.1.1 software. RESULTS :The linear ranges of pyrazinamide and pyrazinoic acid were 25-5 000 ng/mL(r=0.997 6)and 100-12 500 ng/mL(r=0.999 0). The lower limits of quantification were 25 and 100 ng/mL,respectively. Intra-batch and inter-batch accuracy were 92.93%-100.50%,and RSDs of intra-batch and inter-batch precision and matrix effect tests were all lower than or equal to 8.42%(n=6 or n=3). Compared with control group ,tmax of pyrazinamide in trial group was prolonged significantly (P<0.01); there was no statistical significance in other pharmacokinetic parameters between 2 groups(P>0.05). CONCLUSIONS :The absorption of single dose pyrazinamide is delayed with the change of intestinal flora in rats.

ABSTRACT

Objective:To summarize the clinical experiences of 9 ABO-incompatible kidney transplantation at our center and explore its clinical application value.Methods:Methods From April 2016 to December 2019, there were 9 living kidney transplants of ABO incompatible relatives, including type A to type B (n=3), type B to type O (n=3), type B to type A (n=1) and type AB to type B (n=2). Immunosuppressant plus single membrane plasmapheresis (PE) and/or double filtration plasmapheresis (DFPP) and rituximab were employed for pretreating recipients. Adverse reactions of recipients were observed during and after pretreatment. Blood group antibody titer, complications and other related parameters were recorded before and after transplantation before and after monitoring pretreatment.Results:After pretreatment, IgM, IgG and total titer of blood group antibodies were ≤1: 4 on the day of transplantation and the titer of non-blood group antibodies rebounded within 2 weeks (≤1: 8). During preconditioning, 2 patients experienced oral numbness and involuntary dithering during plasmapheresis and there was 1 case of infusion reaction after rituximab dosing. The early recovery of renal function was all excellent. Renal biopsy was performed in 4 patients with slow elevation of serum creatinine and 1 case developed acute antibody-mediated rejection. The survival rate of all recipients at the last follow-up was 100%.Conclusions:Live kidney transplantation of ABO-incompatible relatives is both safe and feasible so that it may help alleviate some shortage of donor kidney.

Objective:To study the different region (DFR) genotyping of 15 strains of Yersinia pestis from Inner Mongolia. Methods:Fifteen strains of Yersinia pestis isolated from Otog Banner and Darhan Muminggan Joint Banner in Meriones unguiculatus plague foci in Inner Mongolia Plateau, Zhengxiangbai Banner in Spermophilus dauricus plague foci in the Songliao Plain in 2019 were amplified by PCR, and identified DFR genotyping. Results:Two strains of Yersinia pestis in Otog Banner jointly deleted DFR loci 1, 6, 7, 13, 15, 16, 17, 18, 23, and 13 strains of Yersinia pestis in Darhan Muminggan Joint Banner and Zhengxiangbai Banner jointly deleted DFR loci 1, 6, 7, 12, 13, 15, 16, 17, 18 and 23. G17 and G20 were classified as genotyping by comparison with DFR locus distribution and genotyping results. Conclusion:The genotyping of Yersinia pestis in the natural foci of Meriones unguiculatus plague foci in Inner Mongolia Plateau and Spermophilus dauricus plague foci in the Songliao Plain is the same as that of the historical strain.

ABSTRACT

Objective To evaluate the efficacy and safety of converting traditional triple immunosuppressive regimen into quadruple immunosuppressive regimen of low-dose tacrolimus plus sirolimus plus mycophenolic acid plus hormone in expanded criteria donor (ECD) renal transplant recipients .Methods A retrospective analysis was conducted for 39 patients undergoing extended standard kidney transplantation with conversion therapy from January 2015 to June 2018 .Renal function ,liver function ,blood lipid ,bone marrow suppression ,positive rate of urinary protein ,positive rate of urinary BK virus and other adverse reactions were analyzed .Results A total of 39 recipients fulfilled the inclusion criteria ,including 28 boys and 11 girls with an average age of (37 .69 ± 11 .07) years and a median postoperative conversion time of 6 months .As compared with pre-conversion therapy ,renal function and estimated glomerular filtration rate improved significantly at 1 month ,3 months ,6 months and 1 year (P<0 .05) .No significant difference existed in the level of blood lipid ,whole blood leukocyte or hemoglobin at pre and post-conversion ( P>0 .05) .No significant difference existed between AST and ALT at pre and post-conversion (P< 0 .05) . After conversion , the positive rates of urinary protein and BK virus urine declined . Subacute rejection occurred in 1 case after conversion .Addition of sirolimus was curative .No severe adverse reactions such as infection and diarrhea occurred in the remaining cases .Conclusions T he quadruple immunosuppressive regimen of low-dose tacrolimus plus sirolimus plus mycophenolic acid and hormone is both safe and effective for ECD recipients of renal transplantation .

ABSTRACT

Objective To evaluate the efficacy and safety of paclitaxel drug-coated balloons (DCB)in treating acute coronary syndrome in patients with high bleeding risk.Methods A total of 35 inpatients with acute coronary syndrome and high bleeding risk who were treated in our hospital from March 2017 to December 2017 were retrospectively analyzed.The patients were divided into the control group (n=20) treated with normal balloon dilatation and the experimental group (n =15) treated with DCB treatment.The age,major cardiac adverse events(MACE)within 3 months,fatal hemorrhage events,all-cause mortality,attributable mortality,target lesion revascularization rate(TLR),coronary artery stenosis degree before intervention,and coronary artery stenosis degree at once and 3 months after surgery,were used for comparison and for statistical analysis between the control and the experimental group.Results There were no significant differences between the two groups in the indexes as the followings:MACE within 3 months after surgery(25.0% vs.6.7%,x2 =2.03,P =0.15),fatal hemorrhage events (10.0 % vs.6.7 %,x2 =0.12,P =0.727),all-cause mortality (20.0% vs.6.7%,x2 =1.24,P =0.27),attributable mortality(10.0% vs.0.0%,x2 =1.59,P=0.21),TLR(14.3% vs.0%,x2 =3.08,P =0.08).However,the incidence of fatal hemorrhage and MACE,all-cause mortality,attributable mortality and TLR were relative low in the experimental group (P>0.05).Conclusions Drug-coated balloon treatment is safe and effective in treating acute coronary syndrome in patients with high bleeding risk.

ABSTRACT

@#This study was to evaluate the cellular inhibition effects induced by anti-AGR2 monoclonal antibody 18A4 in two AGR2-negative melanoma cells, A375 and B16-F10, treated with external oncoprotein AGR2. MTT assay and colony formation assay were performed to detect the cell proliferation rate. Flow cytometry analysis was performed to evaluate cell cycle transition. Cell migration rate was analyzed by wound healing assay. Cell morphological changes were detected by phalloidin staining. The expression of p53 was detected by Western blotting and immunofluorescence. Results showed that 18A4 inhibited the AGR2-dependent tumor properties including enhanced proliferation, accelerated cell cycle, increased cell migration and morphological changes of cells. In addition, by Western blot analysis and immunofluorescence, AGR2 blocking antibody 18A4 also restored p53 expression that was repressed by external AGR2 treated by chemotherapeutic drug doxorubicin. These findings suggest that 18A4 is able to inhibit the cellular tumorigenic properties induced by external AGR2 and is a potential drug against fumor.

ABSTRACT

Objective To illustrate more accurately the feasibility of DNA barcode technology in species identification in different geographical areas.Methods Four plague host animals were selected from three plague natural foci in Inner Mongolia for DNA barcode comparison,GC base content,codon preference and genetic distance calculation,and a phylogenetic tree was constructed.Results Compared between different foci of A llactaga sibirica,the difference of base content was statistically significant (F =5.34,P < 0.05);the variation of Meriones unguiculatus focus Allactaga sibirica base content was larger,and compared with other foci Allactaga sibirica,the difference was statistically significant (all P < 0.05).Different foci between one species rarely exist in codon sites.The genetic distance of intraspecific was less than 1%,and the genetic distance of interspecific was more than 18%.Conclusions DNA barcode of each species is specific,with no interspecific species sharing,and does not change with geographical position.Molecular identification of plague host animal is effective by DNA barcode.

Objective To determine a method for amplification of cytochrome C oxidase subunit Ⅰ (CO Ⅰ) gene of Brandt's vole.Methods 发he Brandt's Voles were captured in Abagaqi Xilingol League Inner Mongolia,and DNA was extracted from liver tissue.CO Ⅰ gene was amplified by nested PCR and sequenced afterwards.Results A band of 657 bp and 1 132 bp was amplified by internal and external PCR primers,respectively,which were consistent with expected sizes.A total of 12 segments of Brandt's Vole CO Ⅰ gene sequences were amplified by PCR and verified by sequencing.一he sequence number was KF182196-KF182207 in GenBank.After gene sequence alignment of the 12 CO Ⅰ gene sequences,it was found that the similarity was 100%,and no base mutation.Conclusion CO Ⅰ gene of Brandt's Vole could be amplified by nested PCR without pseudo gene.

ABSTRACT

The cone cell on the retina of human is the sensor of vision under illumination; it can be classified into three types: red cone cell, green cone cell, and blue cone cell. There is different property of absorbing light for each type of cone cell. In this work, a 10 Hz pulse was used to drive red, green and blue light emitting diodes respectively, and the different monochromatic light with the same luminance was obtained. The eyes of ten subjects were stimulated by different monochromatic light independently; an EGI system with 128 channels was used to record the steady-state visually evoked potential (SSVEP). After applying the fast fourier transform (FFT) to the SSVEP data, we found that the distribution of the neural network in the initial vision cortex activated by the output of the different-typed cone cell remained mainly identical, but there was some difference in intensity between the three types of network: the activity by blue light is the strongest one, that by red light is in the middle, and that by green light is the weakest one.

ABSTRACT

<p><b>OBJECTIVE</b>The purpose of this study was to discuss the principle in neck treatment of cN0 patients with squamous cell carcinoma of buccal mucosa.</p><p><b>METHODS</b>101 patients of squamous cell carcinoma of buccal mucosa at the stage of cN0, who had hospitalized in West China College of Stomatology, Sichuan University from 1980 to 2000, were investigated retrospectively. All the patients received a comprehensive therapy consisting of surgical procedures combined with chemotherapy and radiotherapy. The combining radical therapy of buccal, mandible and neck was the main surgical method. Lymph nodes were cleared after operation and examined by pathological method. The patients had been followed up for more than 3 years.</p><p><b>RESULTS</b>17 patients had lymph nodes metastasis, the occult metastasis (OM) rate was 16.83%. It increased in the high stage of original lesion, the OM rate of T3 and T4 was 18.18% and 52.00%, respectively. The metastasized lymph nodes were mainly located in submandible and superior deep cervix lymph nodes, their respective metastasis rate was 41.18% and 29.41%.</p><p><b>CONCLUSION</b>The rate of occult metastasis of squamous cell carcinoma of buccal mucosa is high, and then we should adopt actively selective neck dissection for the cN0 patients of squamous cell carcinoma of buccal mucosa.</p>

<p><b>OBJECTIVE</b>To investigate the regularity of cervical lymph-node metastasis in oral squamous cell carcinoma patients and determine the treatment principle to cN0 patients.</p><p><b>METHODS</b>1,024 oral squamous cell carcinoma cases who underwent neck dissection between 1980-2001 were investigated retrospectively.</p><p><b>RESULTS</b>The total rate of cervical lymph-node metastasis of oral squamous cell carcinoma was 36.62% (375/1,024), the metastasis rate of cancer in tongue, buccal, gingival, and floor of mouth was 42.82%, 31.93%, 32.76%, and 25.00%, respectively; the rate of occult metastasis was 20.94%(71/339), the incidence of occult metastasis was closely related to the location of primary lesion and T stage.</p><p><b>CONCLUSION</b>The results in this study revealed that the therapy regime of the oral squamous cell carcinoma should be based on carefully clinical examination and compositive analysis on primary lesion related to size, location and cervical lymph-node.</p>

ABSTRACT

Objective: To prepare an immunoconjugate of anti-EGFR monoclonal antibody (ETS) and adriamycin(ADR) and to investigute it's inhibitory effect on human epidermoid carcinoma (A431) in vitro and in vivo and it's side effect on C57BL/ 6J mice.Methods:The immunoconjugate(ETS-ADR) was prepared with an improved glutaraldehyde conjugate method and the cytotoxicity effects of EQ75-ADR on A431 cells and Wish cells were measured by MTT assay. A431 tumor-bearing nude mice and C57BL/6J experimental model were established and were used for testing the inhibitory edicts of EQ75-ADR. The side effect of EQ75-ADR was compared with ADR. Results:The EQ75-ADR showed specific cytotoxicity on A431cells,the ytotoxicity of EQ75-ADR was 50-fold, 14-fold and 10-fold more efficient than that of EQ75, ADR and mixture of EQ75 and ADR (EQ75+ ADR) respectively.The effect of EQ75-ADR on with cells was 10-fold less efficient than that of ADR and EQ75+ADR. As compared with the cytoxicity effect of EQ75-ADR on Wish cells with A431 cells, it showed 300-fold less efficient.Similar cytotoxicity results were shown in 2 h treatment experimental group.
